The Case Manager II provides primary support and case management for individuals with mental illness and co-occurring disorders to help them live independently. They coordinate with PATH programs and local organizations to link clients to housing, treatment, and essential community resources.
Candidates must have a high school diploma or G.E.D. with two years of direct mental health experience, or an associate degree with one year of experience, or a bachelor's degree in social services. A valid driver's license and personal vehicle insurance are required for transporting clients.

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.
What You Bring to the Table (Must Have)
· High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor’s degree in Social Services
· Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
· Willingness to use your personal vehicle to drive clients to appointments and groups, etc.

Forensic PATH teams will coordinate with the other elements of the Trueblood Settlement to conduct outreach in partnership with existing PATH programs and local Homeless Continuum of Care organizations to link individuals to housing, services, treatment and resources. Outreach may occur in a street outreach setting or in a service setting such as an emergency shelter, jail, encampment, or drop-in center.
