As a Hospice CNA/Home Health Aide, you will play a vital role in supporting patients and their families during some of life's most intimate and challenging moments. Every day, you will help create a nurturing, respectful, and comforting environment, ensuring patients feel valued, cared for, and supported.
What You'll Do
- Provide hands-on assistance with daily living activities such as bathing, grooming, dressing, toileting, and mobility.
- Monitor and report changes in patient condition to the hospice nurse or care team, ensuring timely interventions.
- Deliver emotional, spiritual, and compassionate support to patients and their families, fostering a sense of comfort and understanding.
- Help families navigate the hospice process, offering guidance and reassurance during difficult decisions and transitions.
- Maintain a safe, clean, and organized environment for patients, promoting comfort and dignity at all times.
- Document care provided accurately and timely and communicate effectively with the interdisciplinary team.
What We're Looking For
- Compassion, empathy, and patience in caring for patients and their families.
- Strong communication skills to collaborate with patients, families, and the care team.
- Reliability and flexibility to meet the needs of patients in a dynamic care environment.
- Experience as a CNA, HHA, or similar role is preferred, but a heartfelt dedication to caring for others is essential.
- Ability to provide comfort, respect, and advocacy for patients in end-of-life or home health care.

Location and hours
- The job requires driving to where the patient lives which could be a nursing home, an assisted living facility, or a private residence
- The main home office is in Marlborough, MA
- Must have a reliable vehicle
- FULL
- TIME
- Monday -
- Friday 8:30am - 5:00pm
- There is some flexibility on the hours
- Rotating holiday and limited weekend schedule
Responsibilities
- Assist patients with their daily activities
- Care for patient's personal needs
- Provide thorough care
Experience
- Minimum of one year of experience as a Certified Nursing Assistant
- Hospice experience preferred
Skills
- Must be able to read and write in English and follow written and verbal instructions in English
- Demonstrates interest in the welfare of ill and elderly
- Must pass Aide Competency Evaluation Skills
Education
• High School Diploma preferred
