Position: Fee for Service Adult Care Manager
$40.00 per service unit, or $42.00 per service unit for Bilingual Care Managers
$30.00 per hour for all required training
Hours: Up to 20 hours per week with day, evening and weekend hours available
Location: Travel throughout Suffolk County

The Association for Mental Health and Wellness is seeking passionate Fee for Service Adult Care Managers (ACM) to make a difference in the lives of adults who have experienced trauma, significant behavioral/physical health and/or substance abuse challenges.
Assist with navigating complex system of care, build supportive relationships and developing individualized care plans to help people meet their overall health and wellness goals.
The fee for service ACM total compensation per service unit includes travel time, required administrative tasks, and the anticipated time to complete the client services.
Service delivery is community based throughout Suffolk County.
We are seeking candidates that have business day availability.
Candidates must possess a bachelor’s degree in a human service or health discipline and have two years of care management experience. A clean, valid NYS driver’s license and a reliable, insured vehicle are required for travel throughout Suffolk County.
