Position: Adult Care Manager

Compensation: $50,000 to $52,000 per year (based on experience and credentials)

Hours

37.5 hours/week; flexibility in schedule necessary

Location: Riverhead, NY and travel throughout Suffolk County

Start Date: Immediate

Adult Care Managers establish supportive, helping relationships with those who have experienced trauma, significant behavioral/physical health and/or substance use challenges to assist them in achieving improved overall health and social functioning.

Work is done primarily in the community and position entails travel throughout Suffolk County.

Agency vehicles can often be used but a safe, personal auto and a clean NYS Driver’s License are required; mileage reimbursement provided.

Care Managers work in teams and receive weekly supervision, extensive training opportunities and as much support as needed.

Great working environment: laptops and cell phones provided.

Key responsibilities will include, but are not limited to

  • Coordinating and monitoring behavioral, physical health, and social services based on client-driven needs assessment and care plan.
  • Building and monitoring linkages with natural support and formal community resources including hospitals, outpatient clinics, rehabilitation, self-help, and social service providers.
  • Providing supportive, recovery and wellness-directed coaching and care management in-person and via telephone communication.

Qualifications

  • A bachelor’s degree in human service/health discipline and two years’ experience is preferred. Candidates with master’s degrees and previous Care Management experience are strongly encouraged to apply.
  • Those with Bi-lingual skills are encouraged to apply; we strive to achieve a diverse workforce, representative of the communities we serve.
  • Excellent inter-personal communication including the ability to establish empathic, empowering relationships with people who have experienced psychiatric, substance abuse challenges, trauma as well as chronic physical health conditions.
  • Strong communication skills and collaboration with professionals involved in the client’s care.
  • Skilled in excel and various electronic platforms
  • Understanding of the key elements of recovery and wellness, including the role of family and self-help.
  • Sound organizational skills including computer skills (agency uses electronic health records, multiple IT platforms).
  • Basic knowledge of the public health/mental health system, public entitlement and social service programs.
  • Candidate must be a reliable self-starter with the ability to work both independently and as part of an interdisciplinary team.

Flexibility in schedule necessary 37.5 hours per week

Requirements

A bachelor's degree in a human service or health discipline with two years of experience is preferred, though master's degrees are strongly encouraged. Candidates must possess a valid NYS Driver's License and strong communication skills for working with trauma-impacted populations.
