The Economic Support Navigator assists patients and families in accessing public benefits, specifically SSI enrollment and Medicaid redetermination. The role focuses on addressing social determinants of health to improve family economic stability through community and governmental partnerships.
Candidates must have a high school diploma or GED and at least one year of experience working with public benefits like SSI or Medicaid. An associate degree in social work or public health and 3-5 years of experience in population health or social services are preferred.
Associate in social work, public health, community health, or a related field preferred 3-5 years’ experience working in population health, care coordination, social services, or community resource navigation. Familiarity with SDOH frameworks. Direct experience collaborating with SSA, JFS, or similar agencies.
