The RN Case Manager provides direct patient care and manages a caseload in home settings according to physician orders. Responsibilities include performing admissions, developing care plans, and coordinating services to meet patient needs.
Candidates must be a graduate of an accredited School of Nursing and hold a current RN license in Connecticut or a Compact License. A minimum of one year of nursing experience is required.
We are currently seeking an RN Case Manager in Central Fairfield County. The RN's overall responsibility is to provide direct patient care and case management to an assigned caseload in the home setting in accordance with physician's orders. This includes admission of patients for service and determining and developing a plan of care. This position will cover towns in Central Fairfield County region such as Weston, Wilton, Redding and surrounding areas.
Our target pay range for this position is $95,000 to $103,000 per year. This pay range represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
