Provide direct patient care and comprehensive case management for hospice patients in homes and care facilities. Coordinate individualized care plans with an interdisciplinary team to ensure patient comfort and dignity.
Requires a current Connecticut RN license and at least 2 years of nursing experience, with hospice or home health experience preferred. Must have a valid driver's license, CPR certification, and the ability to travel within the service area.
Graduate of an accredited nursing program; BSN preferred Current RN license in the State of Connecticut 2+ years of nursing experience; hospice or home health experience strongly preferred Ability to work independently and respond effectively in crisis situations Strong clinical judgment and interpersonal communication skills Comfortable with electronic health records and mobile documentation Valid driver's license, reliable transportation, and willingness to travel within service area Current CPR certification Must complete RVNAhealth's hospice training program upon hire
