Coordinate activities of the Case Management Department to develop and implement safe, patient-focused discharge plans. Assist with case management referrals, authorizations, and the maintenance of Medicaid state submission forms.
Requires a high school diploma and a current BCLS certificate, with an associate degree or LVN license preferred. Candidates should have knowledge of discharge planning and utilization management terminology, preferably within a hospital setting.
The Hospitalist Nurse Practitioner focuses on providing general care to inpatients of all ages, with utmost patience, care, compassion and understanding. Perform tasks within their scope of practice which may include conducting tests, recording medical histories, providing treatments, assisting physicians and enhancing public awareness on the importance of health, and prescribing medications.
Minimum 2 years' experience working inpatient setting as an NP Hospitalist Days-Saturday and Sunday 7am-2pm (14 hours/weekend) 10-15 patients/shift Assist physicians with admissions and rounding EPIC EMR
Education and Work Experience MSN with specialization as an ARNP. Current state license as an Advanced Practice Nurse. BLS upon hire and maintain current. Two years of experience as a nurse practitioner working on inpatient team. Previous experience with electronic health record.
