Provide comprehensive and continuous health care services to elders and patients within the medical department. Responsibilities include conducting patient visits, interpreting diagnostic tests, and collaborating with an interdisciplinary care team.
Candidates must have completed an accredited NP/PA program and hold certification from the NCCPA or ANCC. Recent experience in geriatric, acute, or sub-acute care is required.

Provide direct medical services to Elders/patients in accordance with medical program policies and procedures and in accordance with practice agreement/supervising agreement. Documentation of Elders/patients clinical condition as required by medical records, third party payors, etc. Provide counsel, support and education to Elders/patients and their families in collaboration with the care plan team. Participate in the on-call schedule for medical coverage evenings, 1 to 2 weekends per year (paid), 1 night of on-call per week (paid) and 1 to 2 holidays per year (paid). Review and interpret x-rays, labs, EKGs, and other diagnostic tests. Enter into a practice agreement/supervising agreement with a physician. Participate as a member of the interdisciplinary care planning team. Visit Elders/patients whenever the medical condition warrants and prepare, authenticate, and date a progress note and authenticate and date all orders. Maintain a schedule of visits appropriate to the Elders/patients medical condition and in compliance with regulations. On his/her scheduled visit will review the Elders/patients total plan of care, including medications and treatments; prepare, authenticate, and date a progress note; and authenticate and date all orders. Arrange for consultation of appropriate medical, surgical, and dental specialties as the needs of the Elder/patient indicate. Follow up initial plan of care developed by physician, provide continuity, ensure understanding and compliance with therapeutic regimen within established protocols and recognize when to refer to physician or another consultant. Serves on committees as necessary. Attend any meetings or conferences, which are generally considered to help maintain their skills with special attention to geriatrics/subacute medicine. Work in collaboration with other health care providers and agencies to provide and where appropriate, coordinate services to the individual. Participate in precepting programs for NP’s/PA’s in training (optional). Assist in approved medical and scientific research studies conducted at St. Ann’s Community Candidate Requirements: Must have completed all courses necessary at an accredited institution offering a NP/PA Program. Must have recent geriatric and/or acute/sub-acute experience. Must have successfully completed all certification requirements and earned the right to use the Physician Assistant Certified Designation from the NCCPA or Nurse Practitioner American Nurses Credentialing Center from the ANCC.
