Conduct comprehensive client assessments and develop individualized care plans for older adults and individuals with disabilities. Coordinate home and community-based services while collaborating with interdisciplinary teams and families to ensure quality outcomes.
Requires current licensure as a Licensed Social Worker (LSW) in the State of Ohio. Candidates must have at least one year of experience working with physical or behavioral disabilities in healthcare or behavioral health environments.
As an LSW Case Manager, you will support long-term clients age 60 and older enrolled in the MyCare Ohio CareSource waiver programs through comprehensive assessment, care coordination, advocacy, and ongoing case management. This opportunity combines community impact, relationship-driven service, and hybrid field-based flexibility while helping individuals remain in the place they call home.
