The Care Coordinator provides education and support to individuals with mental illness or substance use disorders, helping them manage their conditions and develop necessary skills. They also link individuals to community resources and assist in achieving goals outlined in treatment plans.
Candidates must have a degree in a human services field or equivalent experience in delivering services to individuals with mental health or substance use issues. Experience should include providing rehabilitation services, teaching coping skills, and supporting individuals in various life domains.
The services provided by a Care Coordinator (CC) focus on helping individuals and family develop skills, access resources and manage illness to be successful in the living, working, learning and social environments of their choice. CC’s teach, model and practice skills with person(s)-served to promote recovery and wellness.
