The Care Coordinator performs crisis stabilization and coordinates clinical care between internal and external resources for children. They develop service plans, monitor treatment progress, and assist families in accessing community resources.
Candidates must be at least 22 years old with a combination of education and experience totaling 3 years, or a Bachelor's degree with one year of behavioral health experience. A valid AZ driver's license and fingerprint clearance are required.

This position is a Monday- Friday 8am-7pm with one day a week working 10am-7pm, reporting to our Bullhead City Outpatient.
Pay: $20 per hour
