About the Role

• The Home Health and Hospice Aide plays a vital role in providing compassionate, personalized care to patients in their homes or hospice settings, ensuring their comfort and dignity during vulnerable times. This position focuses on assisting patients with daily living activities, monitoring their health status, and supporting their emotional and physical well-being. The aide works closely with healthcare professionals and family members to implement care plans tailored to each patient's unique needs. By offering both practical assistance and empathetic companionship, the aide helps improve patients' quality of life and supports their independence as much as possible. Ultimately, this role is essential in delivering holistic care that respects the wishes and needs of patients and their families in a home or hospice environment.

Minimum Qualifications

  • Minimum age of 18 with a high school diploma or equivalent.
  • Completion of a state-approved training program with current certification as a Home Health/Hospice Aide. Must demonstrate competency in accordance with 42 C.F.R. §484.80 prior to providing independent patient care.
  • At least one year of direct patient care experience in home care, hospice, or institutional setting preferred. Experience in home health and/or hospice strongly preferred.
  • CPR certification required
  • A valid driver’s license, reliable transportation, proof of auto insurance with acceptable driving history is required.

Preferred Qualifications

  • Previous experience working as a Home Health or Hospice Aide.
  • CPR and First Aid certification.
  • Familiarity with electronic health records (EHR) systems.
  • Strong communication skills and cultural sensitivity.
  • Additional training in palliative or end-of-life care.

Responsibilities

  • Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support.
  • Monitor and record patients' vital signs and report any changes in health status to nursing staff or supervisors.
  • Provide companionship and emotional support to patients and their families, fostering a comforting environment.
  • Help with light housekeeping tasks, meal preparation, and medication reminders to promote patient well-being.
  • Collaborate with healthcare team members to follow individualized care plans and ensure continuity of care.
  • Maintain accurate documentation of care provided and communicate effectively with supervisors and family members.
  • Adhere to infection control protocols and safety standards to protect patients and oneself.
