The Registered Nurse will provide high-quality healthcare to Veterans as part of a Patient Aligned Care Team, focusing on patient assessment and care plan implementation. Key duties include triaging urgent patients, coordinating care navigation, and providing patient and family education.
Candidates must have at least one year of experience in Primary Care or Chronic Disease Management and a current, unrestricted state Nursing License. A current BLS certification from the American Heart Association and strong computer skills are also required.
