Provide education on the Medicaid waiver process and create individual support plans based on client needs and goals. Facilitate community-based team meetings and collaborate with service providers to ensure access to necessary supports.
Requires a bachelor's or master's degree in a human services field, or a bachelor's degree in any field with 2 years of direct service experience with elderly or disabled populations. Must possess a valid driver's license, reliable transportation, and a designated home office setup.
