Job description

SUMMARY As a Certified Home Health Aide (HHA), you will provide care to clients in their home, following their plan of care.
You make a positive difference each and every day by demonstrating care and compassion to our patients.
STATUS: Full-Time LOCATION: ElderOne Hudson PACE Center DEPARTMENT: EO HUD HOME HEALTH AIDES SCHEDULE: Days: 7 am - 3:30pm; Evenings: 3pm - 11:30 pm; Nights: 11pm - 7:30 am ATTRIBUTES Compassionate, patient focused and dependable professional Critical thinking skills, decisive judgment and the ability to work with minimal supervision Exceptional documentation and communication skills Ability to read, write and speak English, ability to follow verbal and written instructions RESPONSIBILITIES Patient Care & Service.
Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands Communication.
Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly Documentation.
Accurately and completely document client care on activity sheet Compliance.
Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirements EDUCATION: LICENSES / CERTIFICATIONS: PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

PAY RANGE: $18.65 - $19.75 CITY: Rochester POSTAL CODE: 14617 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant.
It may be modified in the future and eligible for additional pay components.
Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
