Provide habilitation coordination for individuals with intellectual and developmental disabilities residing in nursing facilities. This includes monitoring services, maintaining monthly contacts, and advocating for clients in alignment with trauma-informed and recovery-oriented models.
Requires a Bachelor's degree in Sociology, Social Work, Psychology, Education, or Counseling. A valid driver's license, vehicle, and preferably one year of experience working with persons with intellectual disabilities are required.

Texoma Community Center (TCC) is one of 39 Texas Community Centers, which are governmental entities as defined by Title 7 of the Texas Health and Safety Code.
This position is responsible for providing Habilitation Coordination to individuals with intellectual and developmental disabilities living in nursing facilities and have a working knowledge of all TAC and HHS PASRR program requirements. This includes, but not limited to, TMHP alerts, PASRR Evaluations, maintaining required monthly contacts, linking to communities and resources, coordinating and monitoring services provided by the NF and Local IDD Authority, and advocating in accordance with the philosophy, goals, and objectives of Texoma Community Center. Participates in fidelity reviews as assigned and operates under trauma-informed and recovery-oriented models of care which align with CCBHC standards.
