The Hospice RN Case Manager provides coordinated, patient-centered care and symptom management for individuals with life-limiting illnesses. They serve as the primary clinical liaison between patients, families, and the interdisciplinary hospice team.
Candidates must hold a current Registered Nurse (RN) license in the State of Texas. Hospice, home health, or case management experience is preferred, along with strong clinical assessment skills.

Position Overview The Hospice RN Case Manager provides coordinated, patient‑centered care to individuals and families facing life‑limiting illness. You will serve as the primary clinical guide for your patients—delivering skilled nursing care, managing symptoms, providing education, and ensuring care aligns with hospice philosophy and patient goals.
Essential Responsibilities Complete comprehensive patient assessments and develop individualized plans of care Serve as the primary clinical liaison for patients, families, physicians, and hospice team members Provide skilled nursing visits, symptom management, and compassionate end‑of‑life care Educate patients and families on disease progression, comfort measures, and available support services Coordinate care with physicians, social workers, chaplains, aides, and other interdisciplinary team members Monitor patient status and adjust care plans as needed Advocate for patient wishes and dignity at all stages of care Complete timely, accurate clinical documentation in compliance with hospice regulations
Current Registered Nurse (RN) license in the State of Texas, in good standing Hospice experience preferred (home health or case management experience welcomed) Strong clinical assessment and critical‑thinking skills Excellent communication skills with patients, families, and care teams Ability to work independently while remaining connected to a team Bilingual (English/Spanish) a plus, reflecting the community we serve Compassionate, dependable, and patient‑focused approach to nursing
