Position Summary
The HealthCare Connection is seeking a Case Manager to support our high-risk and complex patient population. In this role, you will provide case management and outreach services, helping patients overcome barriers related to social determinants of health and guiding them toward improved health outcomes and quality of life. The Case Manager will assess patient needs, develop individualized care plans, and connect patients and families with essential community resources. This role requires strong collaboration with an interdisciplinary care team to ensure coordinated, patient-centered care.

Conduct comprehensive assessments of patient and family needs, including physical, psychosocial, educational, and resource-related factors
Assist in coordinating care plans in collaboration with healthcare providers and interdisciplinary teams
Develop and facilitate patient-centered care plans in alignment with Primary Care Providers and Behavioral Health Providers
Ensure effective communication and coordination to avoid duplication of services
Provide referrals and support for mental health and substance use treatment programs
Advocate for patients by assisting with problem-solving, education, and self-management coaching
Schedule and ensure completion of follow-up appointments, screenings, and referrals
Maintain accurate documentation and patient tracking systems, including services provided and outcomes
Build and maintain relationships with community agencies and external resources
Serve as a resource and liaison for patients, families, and the healthcare team
Qualifications
Education
• Bachelor’s degree in Social Work, Sociology, Psychology, or a related field required
Experience & Skills
One or more years of recent case management experience preferred
Experience working with pediatric and adult populations
Knowledge of community resources and social service systems
Experience with Microsoft Office; familiarity with EMR systems preferred
Strong interpersonal, communication, and organizational skills
Ability to work effectively with diverse populations
Bilingual skills preferred
