Direct Home Care - Case Manager Health Services of Northern New York is seeking a Registered Nurse Case Manager

• This position will be responsible for the management of client care in the home setting.
• The Registered Nurse Case Manager will perform nursing assessments, coordinate care with physicians, and have oversite of additional discipline involvement with each client.
• Additional discipline involvement could include Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Worker and Home Health Aides.
• This position will require travel to and from patient homes throughout their specified region.
• An ideal candidate will have excellent communication skills and be comfortable traveling to and working one-on-one with patients in their homes.
