The Care Manager provides patient-centered care coordination, helping patients navigate the healthcare continuum while improving health outcomes and patient experience. This role focuses on chronic disease management, care planning, care transitions, closing care gaps, and connecting patients with community resources.
The Care Manager also addresses Social Drivers of Health (SDOH) and utilizes the SBIRT (Screening, Brief Intervention, and Referral to Treatment) model to support patients with substance use concerns.

Candidates must hold an Associate degree in Nursing, a current RN license, and CPR certification. Clinical nursing experience is required, while a Bachelor's degree and care management experience are preferred.
