The Support Nurse facilitates efforts to relieve suffering and enhance quality of life for patients with advanced serious illnesses. Key duties include conducting phone follow-ups, coordinating care with interdisciplinary teams, and providing patient and family education.
Candidates must hold a current RN or LPN license in Ohio, with LPN preferred. A minimum of two years of experience in hospice and palliative care is preferred.

The Nurse position– will facilitate Reserve Care's efforts to relieve suffering, enhance comfort, promote quality of life and foster choice in the presence of an advanced serious illness for all patients and families. The practice is based on competent clinical knowledge and focuses on clinical practice, education and collaboration related to palliative and hospice care.
