In this role, you will support the Social Services Director in delivering high-quality, resident-centered care. You will play a key role in addressing residents’ psychosocial needs, coordinating services, and ensuring compliance with all regulatory requirements. Key Responsibilities Assist the Social Services Director with care planning and service delivery Support residents and families with transitions, emotional needs, and care-related concerns Complete documentation, assessments, and progress notes in compliance with state and federal regulations Collaborate with interdisciplinary team members to ensure comprehensive care Advocate for resident rights and promote overall well-being Participate in discharge planning and connect residents with community resources Qualifications Required: Skilled Nursing Facility (SNF) experience Knowledge of state and federal long-term care regulations Strong communication, organizational, and documentation skills Compassionate, team-oriented, and resident-focused approach High school diploma required; related certification or coursework preferred Schedule Full-time position Day shift (details discussed during interview)
