Evaluate patient mobility and functional limitations to design and implement individualized treatment plans. Coordinate with interdisciplinary teams and provide education to patients and families to ensure recovery and independence.
Must be a graduate of an accredited Physical Therapy program with a current, unrestricted state license. Valid CPR certification and strong communication skills are required.
As a Physical Therapist, you will play a vital role in delivering high-quality patient care by evaluating mobility and functional limitations, designing individualized treatment plans, and guiding patients toward improved physical function and independence. Your expertise will directly impact patient recovery and quality of life, while also supporting interdisciplinary collaboration by communicating progress to team members and training nursing staff and families in ongoing care strategies. You will ensure compliance with all regulatory and documentation standards, maintain a safe and professional work environment, and uphold the highest ethical and professional benchmarks in alignment with the Company’s values and policies.
Conduct comprehensive patient evaluations to assess physical condition and functional limitations Develop and implement individualized treatment plans to promote recovery and improve mobility Monitor and document patient progress in accordance with regulatory, licensing, payer, and accrediting requirements Communicate patient status, progress, or concerns to supervisors and care team members Coordinate patient scheduling and ensure accurate daily posting of charges to patient records Provide education and guidance to patients, families, and nursing staff on follow-through programs and home exercise regimens Maintain treatment equipment and work area in a clean, safe, and operational condition Adhere to the Company’s Code of Business Conduct, Corporate Compliance Agreement, professional standards, and applicable state and federal regulations Perform other duties as assigned
Graduation from an accredited Physical Therapy program Current and unrestricted Physical Therapy license in the state where services are provided Valid CPR certification Strong organizational, interpersonal, and communication skills Ability to work effectively in a team-based, patient-centered environment
