Description Case managers are an important part of the mental health field. Case managers are there as a safety net for those experiencing hardship and challenges in their life. Case managers provide comfort, suggestions and referrals to outside agencies that will offer the best support or outcome for each situation. The goal of Case Management is to ensure a person is receiving the support they need while working to increase the individual’s ability for self-support and self-advocacy.

Promotes the values of Pearl Health Clinic within the Clinical Departments and the Community Service Provide excellent care to the clients/patients Work collaboratively with others Hope View challenges as opportunities for growth Take steps to promote professional development of staff Solutions Solve problems with ambition and pragmatism Work to achieve organizational goals Inspire Appreciation sparks new ideas Responsibilities – Case Manager Coordinate the delivery of each person's treatment plan Work as a team with the client, their family and other providers to ensure the correct care plan is in place Link client to supports systems that will promote growth Coordinating and monitoring the client's ability to navigate and use services Evaluating progress quarterly while adjusting care as needed to improve care Provide crisis intervention by assessing the situation Ability to be flexible with scheduling and to perform and prioritize multiple functions and tasks Exemplary time management skills Be able to work independently, follow verbal and written instructions, and meet deadlines. Comfortability navigating electronic documents and electronic systems required to perform job duties Maintain strong boundaries and understand cultural diversity Have a strong overview of mental illnesses and their effects on the brain. Ability to maintain confidential information within HIPAA guidelines and organizational policies Adhere to the PRA Ethics and Professional Conduct Value working with underserved and marginalized populations and appreciate diversity Adherence to mandatory abuse reporting laws A mature attitude and sensitivity to populations dealing with mental health challenges Submit records of weekly interventions, initial recovery plans, and quarterly recovery plan reviews in a timely manner Attend monthly individual and group supervision with a clinician Attend periodic team, department, and agency-wide staff meetings Strong communication skills, basic keyboarding skills
