The role involves providing ongoing support to patients transitioning from hospitals and educating them on managing chronic conditions. The nurse will collaborate with a multidisciplinary team to develop individualized care plans and coordinate quality-of-care programs.
Candidates must be a registered nurse with a valid license and proven experience in nursing care methods. Strong communication, organizational, and interpersonal skills are required to work effectively within a healthcare team.
We are looking for an energetic and self-motivated Registered Nurse to join our care team. You will be responsible for providing ongoing support to patients while they transition from the hospital and for educating patients on managing their chronic conditions. The ideal candidate will be able to work collaboratively with a team of doctors, advanced practice providers, pharmacists and dieticians to promote patient well-being.
