RESPONSIBILITIES: Provide resident care services for the facility. Direct, evaluate and supervise resident care. Perform basic nursing skills competently and documents properly. Formulate nursing care plans based on the information gathered and health history. Provide and receive staff report on residents during shift change and review completion of report and throughout assigned shift, noting changes or addition of resident assignments. Assist physician and nursing staff in patient care. Perform physical examinations and diagnostic tests. Communicate and treat residents in a professional, honest, fair, and respectful manner at all times. Verify medication administration accuracy, perform the required checks on the medication administration record and document medications according to policy. Provide care appropriate for the residents served based on demonstrated knowledge of the principles of growth and development over the life span. Assess and interpret data on the residents’ status to identify residents’ requirements relative to their age-specific needs and provides care needed as stipulated in departmental policy. Identify potential infectious process and document appropriately while notifying the Infection Control Nurse and attending physician in a timely manner. Assign tasks and projects to establish an efficient and clean resident care and work environment. Take appropriate action to assure adequate staffing to meet residents needs. Chart all resident care provided in a timely manner according to Hospital District policy. Ensure that all resident care delegated to others is done safely, accurately, and in a timely manner. Transcribe and/or cosign physician’s orders completely and accurately according to Hospital District policy and practice. Conduct resident rounds daily, report problems to the DON and initiate corrective action. Follow facility discharge/transfer procedure and coordinate discharge/transfer with other departments, physicians and ancillary service providers. Accurately document on each shift the nursing resident care activities and the resident’s response to nursing interventions and on observation and charts extraordinary events or changes in resident’s condition. Participate in the health care educational needs for the patient, family, or significant other regarding patient care needs, disease prevention and health maintenance measures. Work with other departments to order services or report changes in orders. Perform patient rounds with physician while obtaining supplies or equipment requested by physician. Administer medications ordered by the physician using prudent nursing judgement and ensuring that all residents receive correct dose, route, time, and drug and document accordingly. Responsible for the proper care and transportation of deceased patients and complete the death certificate according to policy. Notify the medical examiner for residents who expire under unusual circumstances, less than 24 hours after admission, or when foul play is suspected.
