Salary
$30 - $36 / HOUR
Responsible for processing medication orders and maintaining accurate participant medication schedules. Coordinates medication reconciliation during care setting transitions to ensure legal and accurate prescriptions.
Requires a professional nursing degree, a current LPN license, and CPR certification. Must have at least one year of experience working with frail or elderly populations and a valid driver's license.

$30.15 - $36.40 per hour based on experience
M-F 8am-5pm, occasional weekends; does require being a part of the on-call rotation (1 weeknight every 1-2 weeks and 1 weekend every 7-8 weeks)

Senior Community Care of Colorado PACE (Montrose) provides healthcare and supportive services for individuals age 55 and older, helping them live safely, comfortably, and independently in their own homes and communities. As a Program of All-Inclusive Care for the Elderly (PACE) provider and the first of its kind in Western Colorado, our interdisciplinary teams deliver personalized, comprehensive care tailored to each participant’s needs.
