The specialist will assess the health and functional status of members to develop community-based care plans. They are responsible for coordinating services and monitoring the provision of long-term care for older adults and individuals with disabilities.
A bachelor's degree in social work or a related field and a current LSW license are highly preferred. Candidates must have at least two years of professional experience caring for individuals aged 65 and older.
