• Provide comprehensive assessments, evaluations, and diagnostic summaries of chemically dependent patients.
• Compile patient centered, comprehensive treatment plans, meeting the regulatory time frames.
• Document pt.
• Progress towards the goals of treatment within oasas regulatory standards.
• Conduct education and therapeutic treatment protocols focused on pt.
• Need and oasas regulations.
• Provide case management services, in behalf of pt.
• With all collateral agencies.
• Provide necessary reports to tasc, drug court, parole, dcmh, dss, cps.
• Assesses and manages patient risk complete discharge summaries in the oasas time frames, with appropriate aftercare in place.

Pay: $72-75k
