Provide clinical services, psychosocial assessments, and short-term counseling to individuals and families in transitional housing. Coordinate with community providers and agencies to ensure long-term service delivery and crisis intervention.
Must obtain and maintain an LMSW or LMHC license within 90 days of employment. Requires experience with mental illness, substance abuse, and facilitating group curricula for at-risk populations.

Purpose of the Position
The role of the LMSW/LMHC Client Care Coordinator within the social service team is to introduce and provide clinical services to individuals and families living in Win Transitional Housing sites. The Client Care Coordinator is responsible for engaging individuals and/or families in long term services within the community. The LMSW/LMHC is supervised by the LCSW/LMHC Clinical Supervisor.
