The Client Care Coordinator provides clinical services, including psychosocial assessments, crisis intervention, and counseling to individuals and families in transitional housing. They also collaborate with internal and external agencies to develop and implement comprehensive service plans for clients.
Candidates must hold an LMSW or LMHC license or obtain one within 90 days of employment. Experience in facilitating groups, using standardized assessment tools, and working with at-risk populations is required.

Purpose of the Position
The role of the LMSW/LMHC Client Care Coordinator within the social service team is to introduce and provide clinical services to individuals and families living in Win Transitional Housing sites. The Client Care Coordinator is responsible for engaging individuals and/or families in long term services within the community. The LMSW/LMHC is supervised by the LCSW/LMHC Clinical Supervisor.
