The Intensive Care Manager (ICM) provides comprehensive case management services to patients with substance use issues, collaborating with medical staff and behavioral health professionals. Responsibilities include managing patient referrals, developing care management plans, and monitoring patient progress.
Candidates should hold a Master's degree in Social Work, Counseling, or a related field, or be actively enrolled in a qualifying Master's program. Bilingual candidates in English and Spanish are preferred, along with experience in care management for individuals in recovery.

Salary Range: $49,000 – $56,000 annually (commensurate with degree, licensure, and experience)

Job summary: The Intensive Care Manager (ICM) works alongside our medical staff and licensed behavioral health
professionals to provide comprehensive case management services to patients seeking support with substance
use. Services include referral and linkage, individualized care management, and support for adherence to
available treatments, including Medication Assisted Treatment (MAT). The ICM practices within a Substance Use
Prevention and Harm Reduction (SUPHR) framework using culturally sensitive, strength-based, and trauma-
informed approaches.
