• Responsibilities: Assess, plan, and intervene in actual or potential health problems for patients in the ambulatory care setting. Coordinate care for patients, families, and significant others, evaluating their response to treatments and interventions. Perform telephone triage, utilizing clinical knowledge and communication skills to assess patient needs and direct them to the appropriate level of care. Qualifications

Education: AAS or Bachelor's Degree in Nursing (BSN) preferred.

Licensure

Current New York State Registered Professional Nursing license. Current BCLS (Basic Cardiac Life Support) certification. Current Infection Control Certificate. Current Child Abuse Certificate.

Experience

• Minimum of two years of nursing experience. Minimum of one year of nursing experience in an Ambulatory, Medical Surgery, or ER setting preferred. Demonstrated experience with telephone triage and diagnosis procedures. Knowledge, skills, and abilities required to perform this job are typically acquired through a minimum of three years of progressively responsible experience in nursing, including assessing patients.
