The Recovery Care Coordinator supports patients in Medication-Assisted Treatment (MAT) through orientation, ongoing care coordination with providers, and medication access management. The role also focuses on community outreach and grant-funded program development to increase access to care.
A Bachelor's degree in social work, behavioral health, or public health is required, with relevant certifications like CDCA or LSW preferred. Experience in integrated care settings or working with substance use disorders is highly desired.
The Recovery Care Coordinator at Lorain County Health & Dentistry (LCH&D) plays a central role in supporting patients engaged in Medication-Assisted Treatment (MAT) services. This position is responsible for meeting with patients during MAT orientation and providing ongoing support in coordination with Primary Care Providers (PCPs) during follow-up visits. The Recovery Care Coordinator delivers trauma-informed, culturally responsive care coordination to promote patient engagement, adherence to treatment, and positive health outcomes.
This role also supports the growth and development of the MAT program through community outreach, marketing efforts, and partnership-building. This position is grant-funded and focused on increasing access to care, improving retention, and expanding awareness of MAT services within the community.
This position is grant-funded and contingent upon continued funding. The role supports the expansion and sustainability of MAT services at LCH&D and may evolve based on program needs.
