Provide comprehensive physical therapy services to senior adults in their homes according to physician plans of care. Responsibilities include assessing functional status, implementing rehabilitation programs, and supervising Physical Therapist Assistants.
Candidates must hold a degree in the relevant discipline and a current Physical Therapy license in the state of practice. Proficiency in computer use and excellent clinical and interpersonal skills are required.
Provide physical therapy services to patients according to physician's plan of care and as defined in the state Physical Therapy Practice Act. Assess and evaluate therapeutic, rehabilitative/functional status to develop, implement and direct physical therapy programs that focus on rehabilitation. Evaluate the patient’s home environment, assess patients for muscle strength, mobility, gait, ROM potential for rehab. Direct patients and families/caregivers in the use and care of therapeutic appliances. Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient. Identify patient and family/caregiver needs for other home health (PCA) services and refer as necessary. Prepare and submit progress summaries based on the attainment of goals. Supervises PTAs according to organization policy and state regulations.
