Coordinate and complete comprehensive mental health assessments and treatment plans for children in home-based foster care. Collaborate with interdisciplinary teams and families to monitor progress and plan for permanency and discharge.
Requires a Master's degree in Social Work or a related mental health field and at least one year of related experience. Professional licensure (e.g., LMSW, LMHC) and a valid NYS driver's license are mandatory.
Assess children/Families at intake to complete a comprehensive mental health assessment to create a diagnosis and comprehensive treatment plan for recommended service needs. Meet and communicate with biological families, guardians, foster families, and case workers. Provide therapeutic services to identified children/Families based on treatment plan recommendations. Work with an interdisciplinary team to monitor ongoing progress of children and families and update treatment plans. Work with referring and collateral agencies, prepare reports, and correspondence. Maintain Clinical case records observing Agency, OMH, and other state mandates. Diligently plan for discharge/permanency, at the time of admission.
