Coordinate medical, dental, and mental health services for youth and adolescents with emotional and physical needs. Manage medications, perform nursing observations, and maintain documentation using EHR systems.
Must be a Licensed Practical Nurse (LPN) in New York State with CPR certification and 1 to 2 years of experience. Ability to pass a physical and complete TCI training is required.
Complete the nursing observations report findings to the registered nurse Review lab and test results, reporting critical values to RN and providers Medication management including ordering medications and obtaining prior authorizations Dispense medications Communicate with medical director/providers regarding observation in client’s health/behavior Provide client and family education regarding medications and treatments Collaborate with our staff regarding concerns and behaviors Participate in Interdisciplinary Medication Review meetings Nursing documentation using EHR Update discharge planning needs Other duties as assigned within scope of practice and based on agency needs Requirements and Education: LPN Licensed in New York State CPR Certification required. 1 to 2 years of experience Experience working with youth and adolescent population desired Must pass physical. Must be able to complete TCI training.
