Provide person-centered case management and housing stabilization support for unsheltered adults using the Housing First model. Develop action plans and maintain community relationships to help clients achieve long-term housing and financial independence.
Requires a bachelor's degree in human services or social work (preferred) and 2 years of case management experience or lived experience with homelessness and mental health. Must possess a valid Massachusetts driver's license and a personal vehicle.
Working onsite at the office and in the field with flexibility to respond to needs as they arise throughout the day Engage in client-centered/harm-reduction/Housing First framework. Forge and maintain positive/ productive working relationships with community members and coalitions Advocate to ensure that client interactions with support services and interventions are as person centered/strength based/recovery oriented Collaboratively develop and implement needs assessments and action plans with clients that improve their success in sustainable housing Address ongoing clinical and/or systemic needs – with attention to potential risk factors Develop and implement safety response plan as needed Working knowledge of area resources and providers is a plus. Familiarity with the challenges related to homelessness, substance use, mental illness and trauma is a must.
