Collaborate with the CSA team and families to develop and implement measurable goals within Individual Care Plans. Support caregivers through coaching, modeling, and advocacy to help them access community resources and behavioral health services.
Requires an Associate's degree in human services or a related field and a valid driver's license with reliable transportation. Candidates must have lived experience as a parent or caregiver of a youth with behavioral health needs.
Work closely with the CSA team, family, and other collaterals in developing measurable family partner goals to support the family’s Individual Care Plan (ICP) and related goals. Engage caregiver(s) in activities in the home and in the community that are designed to address one or more goals/tasks on the family’s ICP. Assist caregiver(s) in meeting the needs of the youth through educating, supporting, coaching, modeling, advocating and guiding. Teach caregiver(s) how to network/link to community resources and treatment providers. Support caregiver(s) in advocating for services and resources to meet youth and family needs. Guide, support and foster empowerment including, linkages to individual, peer/parent support and self-help groups.
