About the role

The Registered Nurse Case Worker supports the physical and mental well-being of agency consumers through direct care and case management. They collaborate with physicians, administrative teams, and families to ensure high-quality care and maintain detailed records.

Our Registered Nurse will provide case management, skilled nursing delegation, training and consultation for a skilled nursing case load in a community-based setting.

The Registered Nurse will apply energy and skills to support and preserve the physical and mental well-being of the agency consumers.

You will collaborate with physicians, administrative team members, direct-care staff and the client's families/guardians to ensure quality care to the agency consumers.

Please note

This role requires driving, therefore a "clean driving record" is critical.

  • Minimum education, training, requirements and prerequisites
  • RN license, CDDN preferred.
  • Medication Trainer certification or ability to acquire.
  • Understanding of the code of nursing regulations.
  • Understanding of the Nurse Practice Act
  • Skilled Nursing delegation.
  • Competent nursing procedures and hands-on skills.
  • Valid Drivers' License and proof of insurance.
  • Satisfactory Driving Record in compliance with Driver Requirement Policy.
  • Access to a personal vehicle for individual transport as required.
  • Knowledge of and ability to comply with state licensing, Medicaid and other agency standards.
  • Ability to maintain records, prepare reports and complete required paperwork according to licensing and agency requirements in a timely manner.
  • Flexibility to work at sites other than usual work location when circumstances warrant.
  • Schedule: M-F, Daytime hours with rotational on call responsibility.
  • DRIVING SPECIFIC (The following list may or may not be all inclusive at time of posting):
  • Must not have any of the following violations within the last 5 years:
  • Reckless driving, including speeding 20mph or more over the posted speed limit.
  • Driving under the influence/impaired/intoxicated or refusing a blood/breath test.
  • Manslaughter or negligent homicide.
  • Driving after being declared a habitual offender.
  • Driving on a suspended or revoked license
  • Driving a commercial motor vehicle while disqualified, under the influence, with a blood alcohol level of .04 or higher, refusing a blood/breath test, or violating an out-of-service order.
  • Injuring a person while racing - felony.
  • Failure to stop at the scene of a crash where there is injury, death, or property damage of $1000 or more.
  • Emergency vehicle violation - death.
  • Vehicular assault or willful stopping, impeding, or damaging a vehicle.
  • Blocking access to the service facility.
  • Attempting to elude police.
  • Operating an unsafe vehicle.
  • Vulnerable Road User – Death.
  • Must not have any of the following violations within the last 3 years:
  • Passing a stopped school bus.
  • Careless driving.
  • Three or more minor moving violations within the last three years.

Requirements

Candidates must hold an RN license and a valid driver's license with a clean driving record and access to a personal vehicle. Knowledge of nursing regulations, the Nurse Practice Act, and Medicaid standards is required.
