Deliver comprehensive case management services to prenatal, postpartum, and parenting families to improve access to medical care and community resources. Responsibilities include conducting home visits, developing individualized care plans, and providing evidence-based education on healthy behaviors.
A bachelor's degree in Social Work, Psychology, Sociology, or a related field is preferred, along with a valid driver's license. Preferred candidates have at least two years of case management experience and familiarity with community resources in Dauphin County.
The Grant-Funded Healthy Start Case Manager is responsible for delivering comprehensive case management services to eligible prenatal, postpartum, and parenting families enrolled in Hamilton Health Center’s Healthy Start program. Healthy Start serves pregnant women and families with children up to 18 months of age who reside in Dauphin County. The Case Manager works to improve access to medical care and community resources, promote healthy behaviors before, during, and after pregnancy, and support safe and healthy infant and toddler care practices through a patient-centered, community-based approach.
