The Physical Therapist evaluates and provides skilled therapy services to help residents restore strength, mobility, and functional independence. This includes developing individualized treatment plans and collaborating with an interdisciplinary care team to ensure high-quality outcomes.
Candidates must hold a current Physical Therapist license in California and be a graduate of an accredited Physical Therapy program. Strong communication and documentation skills are required, while SNF experience is preferred but not mandatory.
The Physical Therapist is responsible for evaluating, planning, and providing skilled physical therapy services to help residents restore strength, mobility, balance, and functional independence. This role works closely with the interdisciplinary care team to ensure high‑quality outcomes. Responsibilities Conduct comprehensive evaluations and develop individualized treatment plans for residents with physical and functional impairments. Provide therapeutic interventions including gait training, strengthening, balance activities, pain management, and mobility training. Document evaluations, daily notes, progress reports, and discharge summaries in accordance with regulatory and facility standards. Collaborate closely with nursing staff, therapy team, physicians, and families to support resident goals. Educate residents and caregivers on exercise programs, safety techniques, and mobility strategies. Participate in interdisciplinary care conferences and contribute to overall care planning. Maintain compliance with state, federal, and professional practice regulations. Qualifications Current Physical Therapist (PT) license in the state of California. Graduate of an accredited Physical Therapy program. Strong communication, documentation, and time‑management skills. SNF experience preferred but not required—new graduates are welcome!
