The RN Care Coordinator manages oncology patient outcomes by assessing clinical needs and facilitating complex treatment plans across the continuum of care. They focus on ensuring continuity of services, enhancing quality of life, and providing specialized IV/VAD care in an outpatient setting.
Candidates must have an Associate Degree in Nursing, current Minnesota RN licensure, BLS certification, and at least one year of acute or oncology nursing experience. A Bachelor of Science in Nursing and Oncology specialty certification are preferred.
Bachelor of Science in Nursing Oncology specialty certification - Oncology Certified Nurse Previous case coordination experience One-year clinical experience in the Oncology care setting.
