The Nurse Navigator coordinates care for patients requiring follow-up after diagnostic mammography and lung screenings. They facilitate timely appointments and serve as a link between providers and community services to improve patient outcomes.
Candidates must be a Registered Nurse licensed in Oklahoma with a Bachelor's degree and BLS certification. 3-5 years of relevant experience is required.
Under the general supervision of the Director of Radiology, the Nurse Navigator is responsible for coordination of care across the continuum for patients that have been identified as needing follow-up appointments after diagnostic mammography and low dose lung screening exams. This position is responsible for the overall coordination of care for the individual by facilitating timely appointments and communication between providers. This position serves as a link to community services and works collaboratively with other healthcare team members to help achieve goals of improved patient care, experience, and outcomes.
