Position Summary

The Senior Integrated Health Care Clinician provides clinical and case management services for children and adults across the lifespan in an integrated health care setting that provides primary care, behavioral health and dental services. The Senior Integrated Health Care Clinician will foster positive working relationships with co-workers, clients and external key stakeholders. $10,000 sign on bonus available! Half paid at start the other half paid at 1 year of service! Federally Qualified Health Center (FQHC) in Plainville, CT. Eligible for up to $250,000 in loan repayment through National Health Service Corp (NHSC). SALARY $68,000.00 EDUCATION AND EXPERIENCE/QUALIFICATIONS Master’s degree in Marriage and Family Therapy, Counseling, Social Work or related license eligible field or a doctoral degree in Psychology is required. Conducts program specific screening and evaluation and formulates diagnosis in accordance with the Diagnostic Statistical Manual (DSM). Develops and documents, clear clinical formulations and treatment plans that are individualized and measurable, developed in collaboration with the client/family and others as relevant, using behavioral health screening as directed, and are inclusive of behavioral health, primary care, and dental goals as indicated. Demonstrates knowledge of psychopathology and primary modalities of psychotherapy, including individual, group and family. Correctly triages clinical emergencies, including suicide and danger to others. Uses best practices and evidenced based practice models for psychotherapy where appropriate. Makes effective use of topic oriented, focused, time-limited group therapy.
