About the Role
SJCH is seeking a Family Nurse Practitioner (NP) to provide comprehensive, whole person care to patients. In this role, you’ll advance our mission of health equity by delivering high quality, community based primary care in a collaborative, interdisciplinary environment.
You’ll work alongside medical, dental, and behavioral health professionals to deliver integrated services that make a real difference in patients’ lives every day.
What You’ll Do
Provide comprehensive primary care for patients across the lifespan
Conduct physical exams; order, interpret, and manage diagnostic testing
Diagnose and manage acute and chronic medical conditions
Collaborate with multidisciplinary teams to deliver integrated, whole person care
Educate patients and families on disease prevention, treatment plans, and healthy lifestyle choices
Document patient care accurately and timely in the EHR system
Support SJCH’s mission through clinical service, outreach, and community engagement
Schedule
Full‑time, 5‑day workweek
Includes two Saturdays per month as part of the regular schedule
When a Saturday is worked, a weekday off is provided during the same week
One evening clinic per week, scheduled from 11:00 a.m. to 7:30 p.m.
