Requirements summary
Requires a high school diploma or GED, with a bachelor's degree preferred and two years of experience in human services, public health, or street outreach. Proficiency in English and Spanish is strongly preferred.
The Housing Care Manager is responsible for providing direct support and housing navigation services to transgender-identified clients and unaccompanied immigrant minors who have been victims of crime in the state of California. The Housing Case Manager works collaboratively with full-time, Housing Care Managers who assess client’s situation, resources, needs, and work with them to develop Housing Stability Plans, link them to support services, and assist with CA victim compensation claims. Housing Care Manager plays critical role in supporting vulnerable populations such as low-income families, individuals experiencing homelessness, and those with disabilities in accessing safe, affordable, and sustainable housing options.

Qualifications
Education & Experience
High School diploma or GED required; Bachelor degree in related field preferred;
Two years’ related experience within human services and/or public health;
Two years’ experience conducting community and/or street outreach, specifically to hard-to-reach populations;
Experience in community organizing.
Experience working in underserved communities such as undocumented workers, unaccompanied minors, LGBT populations, communities of color, youth/adolescents, and those experiencing homelessness, substance abuse, and/or mental illness; and
Conduct assessment to determine client needs, develop care plan, and provide ongoing case management.
Establish and develop working relationships with clients, landlord, property managers, immediate team members, and staff within, and outside of SJCH.
Provide comprehensive education and information through street outreach, health fairs, school relationships, community events, and other approved activities.

Attend regularly scheduled and impromptu meetings and maintain communication with program team members and supervisor.
Assist in the production of required weekly, monthly, and/or quarterly data collection reports.
Maintain outreach activity calendars and logs according to program standards.
Assist clients with applying for public benefits, victim compensation, and any other type of relief they may be eligible for.
Attend appropriate community resource meetings and trainings, as assigned.
Work in collaboration with other departments when required.
Create, edit, and reproduce outreach materials.
Identify appropriate housing programs/resources.
Submitting housing applications and searching for permanent housing.
Navigate The California Victim’s Compensation Board (Cal VCB) website to assist victims of crime with compensation and/or restitution.
Provide trauma-informed, crisis counseling and support - with linkage to higher-tier level services as needed and coordinate transportation.
Accompany victims to court, police stations, and assist with completion of applications for restraining orders.
Advocate on behalf of clients to employers and landlords.
Partner with community-based agencies, landlords, and housing agencies.
Attend meetings, trainings, and other work-related events as needed.
Other duties may be assigned or may be modified as business needs dictate.
