Ambulatory Care Nurse II

Overview

• The Ambulatory Care Nurse (ACN) is a Registered Nurse responsible for providing nursing care management services to the assigned populations.
• The ACN utilizes a collaborative approach to anticipate, integrate, and coordinate the patient?s plan of care and maximize the ability to remain in a community setting.
• In collaboration with the physician, the nurse identifies at risk patient populations who require care management services.
• The ACN serves as a patient/family advocate and acts as a communication link between the physician, patient, family, and interdisciplinary health care team, as well as community resources.
• The Ambulatory Care Nurse understands the needs of the organization and supports the mission, values, and management of patient care services.
• The ACN actively supports and incorporates ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and WakeMed's Patient & Family Centered Care (PFCC) model.

Registered Nurse Required Education: Diploma Nursing Or Associate's Degree Nursing Required Bachelor's Degree Nursing Preferred Experience
