The Ambulatory Care Nurse provides nursing care management and coordinates patient plans of care to maximize community-based living. They act as a primary communication link between physicians, families, and interdisciplinary healthcare teams.
A Registered Nurse license is required, along with a Diploma or Associate's degree in Nursing, though a Bachelor's degree is preferred. Candidates need 2 years of nursing experience, or no experience if they possess the preferred education.
