The Case Manager coordinates care across multiple providers to ensure clients receive support identified in their individualized treatment plans. They are primarily responsible for overall discharge planning, aftercare placement, and maintaining communication with the treatment team and referral sources.
A high school diploma is required, while a bachelor's degree in Social Work, Counseling, or a related human services field is preferred. Candidates should ideally have experience in behavioral health settings and case management, along with CPR and de-escalation certifications.
